please download the form (attachment 1) and fill it out and print it
then submit the form of hard copy to B1-500(11) before 21st, Feb.
After or missing the deadline, the application form will not be accepted.
The application form for doubting score in other semester will not be accepted.
The application form is only for bachelors.
